Position Summary:

 

The Licensed Practical Nurse (LPN) Supervisor is responsible for assisting in the overall operation of the Assisted Living and Memory Care departments.

 

 

Required Experience for LPN Supervisor:

  • Currently licensed LPN for the State of Ohio with license in good standing.
  • Assisted Living experience preferred.
  • Dementia care experience preferred.
  • Ability to communicate effectively.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to respond to staff, resident, and family concerns in a professional and timely way.
  • Flexibility to work nights, weekends and holidays required for this role.

 

Primary Responsibilities for LPN Supervisor:

  • Assists in providing compassionate and competent care for Assisted Living and/or Memory Care residents.
  • The LPN Supervisor supervises staff on shift; oversee their job performance to meet the needs of all residents.
  • Develops and maintains positive relationships with residents and their families.
  • Ensures all resident personal care is provided to the highest standard and in a private and dignified manner.
  • Manage medication administration to include:
  • Ordering and receiving of medications
  • Processing physician orders
  • Review MAR’s and reconcile discrepancies with pharmacy.
  • Ensure all resident medication are available for delivery.
  • Respond in a timely manner to resident needs as observed, reported by staff, written communication, resident or family – Report to Wellness Director
  • Review occurrence reports – incident reports – report to Wellness Director
  • Maintains appropriate and complete documentation per company policies.
  • Maintains and updates Care Associate task sheets as needed.
  • Input information into Yardi
  • Presents professional appearance/dress.
  • Available to work weekends as necessary as assigned by Wellness Director.
  • Available for on call as assigned
  • Perform other duties as assigned
  • The LPN Supervisor works toward continual improvement of the overall organization.

 

General Working Conditions:

This position entails standing for long periods of time.  While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects.  The employee is occasionally required to reach with hands and arms.  This position requires the ability to review detailed documents and read computer screens.  The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate.  Occasional travel to different locations may be required.
